Please use the link above to access the SEEK Trend Data Form. As a result of new enchancements to the SEEK application at the Kentucky Department of Education, beginning October 31, 2008, some forms that were traditionally submitted to KDE on paper will be replaced by a web-based counterpart. To access the forms a user name and password must be obtained from your district's Web Administrator.
